Human-in-the-loop AI is a design pattern in which human judgment is integrated into an AI workflow at defined decision points — reviewing low-confidence outputs, correcting errors, or approving high-stakes actions. It combines the scale of AI with the accuracy and accountability of human oversight.

  • Confidence-triggered escalation. Low-confidence turns automatically route to humans in real time.
  • Customer-triggered escalation. Customers can request a human, and the system honors it with full context transfer.
  • Feedback capture. Human corrections are structured as training signal that updates prompts, rules, and models.

  • Human correction rate — frequency of human edits to AI output.
  • Time-to-human — latency between confidence drop and human involvement.
  • Feedback loop closure rate — percentage of human corrections that actually update the AI system.
  • Downstream accuracy improvement — measurable reduction in errors after human-loop corrections are integrated.
  • Reviewer workload — distribution and sustainability of human review load.

How to improve performance

  • Escalate by risk, not by volume. Route humans to the turns where their judgment actually matters.
  • Close the feedback loop. Human corrections that do not update the system are wasted effort.
  • Give reviewers context. Humans reviewing AI decisions need the same context the AI had, not a stripped-down summary.
  • Design for reviewer wellbeing. Continuous review of edge cases is cognitively taxing — rotate, sample, and support reviewers.
  • Enforce output control on compliance turns. Some turns should not be escalated at all — they should use deterministic responses.
  • Measure reviewer calibration. Two reviewers should agree on most cases; persistent disagreement signals unclear policy.
